Business inquiries collect your name, organization, email, optional phone, topic and message. Referral requests collect your organization, name, role and direct phone. Call-back requests collect a name and phone number. Referral and call-back requests include permission to contact you. Every form includes a hidden field that discourages automated submissions.
How the information is used.
Gateway uses these details to review your inquiry and contact you. Form submissions are delivered to [email protected]. Gateway does not sell website form submissions.
Medical records and sensitive information.
Please do not include patient names in referral requests, or diagnoses, wound descriptions, photos, medical records, insurance numbers or Social Security numbers in these forms or in ordinary email. Contact Gateway for instructions on sharing clinical information securely.
Care and availability.
An inquiry does not create a clinician–patient relationship, confirm acceptance for care or schedule a visit. Care is provided by independent partner practices, which give their own privacy information for a treatment relationship.
Technical information.
Hosting and delivery services may process IP addresses, browser information and request logs to operate and protect the website. This website does not load advertising or analytics tags.
