Before you call

• Have your organization, direct contact, the patient’s city or ZIP, care setting and expected discharge timing.
• Identify the receiving facility or home contact and the clinicians already involved.
• Ask how to send discharge notes, current wound orders and other records securely.
• Confirm acceptance, visit availability and the next responsible contact before finalizing follow-up.
• Keep patient names, diagnoses, photographs and records out of the public web form.
