We discuss the care need, explain which records and orders are needed and how to send them securely, and confirm what can be arranged for the patient’s location.
Care is scheduled.
Visits by a partner clinician are planned around the care plan. Findings and next steps go to your team, the attending physician and the specialists involved.
Advanced care, coordinated.
When the treating clinician orders them, Gateway coordinates advanced options such as negative pressure wound therapy, compression, and skin substitutes and other cellular and tissue-based products, along with the supplies that go with them.
The plan stays with the treating clinician. We make sure the facility, the family and the supplier are working from the same orders.
