Questions and observations to share with the care team when a loved one is at risk for a pressure injury or bed sore.
What to discuss with the care team
- Pressure injuries are more likely when a person has difficulty moving, especially over the heels, hips and tailbone.
- Skin changes may include lasting discoloration, unusual warmth, firmness, tenderness or an open area.
- Ask the care team to explain the plan for mobility, pressure relief, skin checks and support surfaces.
- Seek prompt medical advice for worsening pain, pus, fever or hot, swollen skin around the wound.
