Pressure injuries can develop when skin stays under pressure, especially when movement is limited. Care considers the wound alongside mobility, comfort and the support available at home or in a facility.

- Skin changes over heels, hips, the tailbone and other pressure areas
- Mobility and the current pressure-relief plan
- Support surfaces and repositioning
- Nutrition and other factors that affect healing
- Shared follow-up with caregivers and nursing teams
- Supplies for the ordered plan
In facilities and at home.
Partner clinicians work with your nurses or caregivers on the plan for pressure relief, skin checks and dressing care, and the plan follows the patient if they move to another setting.
For referring teams, patients and families.
Facilities, discharge teams and physician practices can refer a patient. We coordinate follow-up with the existing care team.
Patients and families can call with the city or ZIP and care setting. We will explain what records or orders are needed before a visit.
